To Request Your Student Cards
Complete the Outreach Training Report Form.
You will need to know the UCSD Personal IDentification number (PID) you were given when you registered for your trainer course at the UCSD OSHA Training Institute. If you do not know your PID number, you can call 800-358-9206 to speak to a representative.
Construction Industry and General Industry
New standards have been released by OSHA. Please review these significant changes. All trainers must download and review the following documents:
For trainers submitting their first 10 Hour or 30 Hour Construction or General Industry Student Card request to UC San Diego OSHA Training Institute you will need to submit:
- A New Trainer Report Form
- A list of all students’ names
- A list of course topics and time spent on each topic
- A copy of your trainer certificate or card

3 Ways To Submit Your Requests — Email, Fax or Mail
Student Card Requests can be submitted via email, fax, or mail. Outreach Trainers must send requests for student cards to the OSHA Training Institute Education Center where they took their last trainer course (OSHA 500, 501, 502, 503, or 5600). Please allow up to four weeks for delivery of your cards.

Replacement Cards/Certificates:
Replacement Cards
Replacement cards are available for both trainers and students in case of loss or damage. Trainers can qualify for a replacement card if they have taken the OSHA 500, 501, 502, or 503 from the UCSD OSHA Training Institute within the last four years.

OTI Outreach Updates
May 1, 2009 - Federal OSHA has placed new restrictions on the distribution of OSHA student cards. Per Federal OSHA guidelines, UCSD's OSHA Training Institute Education Center will now provide Outreach Trainers with exactly the number of cards reported; Outreach trainers will no longer receive "extra" cards.
