Student Card Requests for:

OSHA 10 Hour and 30 Hour Construction

OSHA 10 Hour and 30 Hour General Industry

OSHA 7600 - Disaster Site Worker

Requests will be accepted via mail, email or fax. Please allow up to four weeks for delivery of your cards. 

Trainers who took their last Construction or General Industry Trainer Courses at two different Education Centers will have to send their requests to two different Education Centers.

Program Administration details:

For students submitting their FIRST 10 Hour or 30 Hour Construction or General Industry request to UCSD:

  1. The Personal Identification Number (PID) you were given when you registered for your Trainer Course at the UCSD OSHA Training Institute.
    (Call 800-358-9206 to obtain this information)
  2. You must use the name you used to register at UCSD Extension; nicknames will not be accepted.
  3. OSHA Outreach Training Program Report (LONG FORMAT)
  4. Students' names.
  5. List of course topics and time spent on each topic.
  6. Provide a copy of your trainer certificate or card.

For students who have previously submitted 10 Hour or 30 Hour Construction or General Industry requests to UCSD:

  1. The Personal Identification Number (PID) you were given when you registered for your Trainer Course at the UCSD OSHA Training Institute.
    (Call 800-358-9206 to obtain this information)
  2. You must use the name you used to register at UCSD Extension; nicknames will not be accepted.
  3. OSHA Outreach Training Program Report (SHORT FORMAT)
  4. Students' names.

Requests for Disaster Site Worker cards require the following information:

  1. Disaster Site Worker Outreach Training Program Report-including complete Training Certifications and Information section.
  2. Download the Disaster Site Outreach Training Report. (All formats are available; full mail format, email format, fax format and short mail format.)
  3. Students' names.
  4. A copy of your course certificate or trainer card if you do not have an ID number.

Submissions must contain all the information identifed above.  Pleae allow 4 weeks for delivery of cards.
